Job Status: Part Time (20-Hours)
Description: The PT Chronic Disease Patient Navigator is responsible to oversee the health center’s activities related to providing high quality standards of care to patients diagnosed with diabetes, asthma, and/or hypertension. Responsibilities include, but are not limited to, the following: Using electronic patient medical records, and information provided by insurers, identify patients with chronic diseases who have not received required/recommended services; Contact and schedule patients for services. Arrange transportation as needed; Participate in the development and ongoing review of health services to patients with chronic diseases, including in-house referral (i.e., nutrition, optometry) and external referral (i.e., exercise programs); Work with the chronic disease nurse case managers to assure patients with chronic diseases are educated about their disease(s) and have self-management plans in place. Assist patients in achieving their self-management goals; Coordinate the development and distribution of patient education materials for health center users and members of the community.
Qualifications: HS diploma or equivalent required. AS/BA/BS within a related healthcare field preferred. Bilingual in English/Spanish or English/Portuguese required. Prior healthcare work experience demonstrating the ability to understand and present simple concepts of disease management required. Excellent interpersonal skills and ability to connect with patients who are ethnically and racially diverse required.
